Terms & Conditions

In consideration of using CHALK Private Wellness Club and the premises, the customer agrees that: (i) Neither the Club, the Studios, the Company or the directors, employees or agents shall be liable for any loss, damage or theft of personal property belonging to the client or their guests upon the Studio premises. (ii) Neither the Club, the Studio, the Company or their directors, employees or agents shall be responsible for any death or injury, occurring upon the Club and the Studio’s premises or as a result of the use of facilities, food, beverage, supplements and/or equipment provided by the Club or Studios, except to the extent that such death or personal injury arises from any negligent act or negligent omission of the Club, the Studios, the Company or any agent or employee of the Studio or the Company.
Liability Waiver CHALK (“the Company)” Pay As You Go (“PAYG”) Annual Membership ("AM"), Annual Membership (Monthly) ("AMM") & Monthly Membership (“MM”) Agreement
The Agreement
1. Waiver Form
You agree to read the Waiver & Par-Q form at the bottom of these terms, confirming that you are in healthy condition and have no medical conditions that prevent you from exercising. 

2. Minimum age
You understand and agree that the minimum age for use is 16 years old. Customers under the age of 18 are required to provide certification from a parent or legal guardian confirming understanding and acceptance of the risks posed.

3. Use of Image
You understand that during your time at CHALK, photographs and/or videos may be taken in which you may be identifiable. You understand and agree to allow CHALK and partners to take photos and videos of you for promotional purposes.

4. Fees
Will apply for:
 PAYG late cancellations. If you cancel your booked sessions within our 24-hour cancellation window, your session cost will be charged.

5. Refunds 

Refunds will only be provided if you cancel your booking more than 24 hours from the booking time. 
6. Payments
Annual Memberships (“AM”), Annual Membership (Monthly) ("AMM") Monthly Memberships (“MM”) & Pay as You Go (“PAYG”): Payments are made in advance.

"AM" are billed from the sale date, in 1 instalment that will last for 12 calendar months, "AMM" are billed from the sale date, in 12 monthly instalments that will last for 12 calendar months, “MM” are billed on a rolling membership per calendar month, on the same date of every month. You understand and agree to provide CHALK with permission to securely store your card details on file and authorise CHALK to charge your card for Membership fees or account or account payments you make.

7. Your Health
You confirm that you are in good physical health and you know of no medical or non-medical reason as to why you cannot or should not partake in active or passive exercise.
 You confirm that you are aware of any risk that exercise may carry, and acknowledge that you are participating willingly.
 You acknowledge that it is your responsibility to seek clear professional medical advice if you have any prior health concerns. You recognise that CHALK or any of their staff, contractor(s) or partners is able to give any medical advice to you.

8. Indemnity
You agree to fully indemnify without limitation that any trainer(s), partner(s), agent(s), director(s) or contractor(s) against any injury, malpractice, mishap, or accident during any and all interactions with CHALK away from, or at the primary place of business for CHALK.

9. Responsibility and Paying for Your Account
You can acknowledge and agree:
- To make sure that your bank account can accept direct debits and ensure that there is enough money in your account on the payment day for CHALK to collect the full balance owing.
- To notify CHALK if you are transferring or closing your account at least 10 days before the next direct debit.
- To notify CHALK of any changes to your card, including expiry date and number at least 10 days prior to the next direct debit.
- To make sure that the payment method on file remains valid for the full length of CHALK membership or usage.
- If your payment method fails, you accept all responsibility and are liable for any resulting fees. After 7 days of non-payment from your billing date CHALK will suspend all access to CHALK until the outstanding balance has been made in full. You understand and acknowledge that CHALK will continue to direct debit your account for the full outstanding balance without notice until the balance has been settled. CHALK will make reasonable effort to advise you on the status of your failed payments via auto-emails.

10. Changes to Your Agreement
CHALK have the right to add, change or remove some terms and conditions. This may include changes to opening hours, services and facilities available at CHALK and fees. The most up to date membership fees will always apply. CHALK will always notify you as far in advance as possible when changes are to be made.

11. Increases to fees
-We reserve the right to increase your fees with 30 days written notice, thereby giving you the opportunity to make any changes to your account, if necessary.

12. Cancellations / Terminations / Freeze / Re-start / Refunds
You acknowledge and agree:

- "AM" can be paused for up to 4 weeks in the 12 month period. This time will be added onto your 12-month term.
- "AM" & "AMM" cannot be cancelled and will expire 12 months after the sale date.

- Intro offers are non-refundable at anytime.

- "MM" there is no cancellation fee to terminate, re-starting "MM" after cancellation results in the loss of any previous promotional rates and you will be charged a £50 re-start fee.

- Cancellations will be processed on receipt of a 30 days written cancellation request to

- Cancellations can only be processed in-line with your billing date, pro-rated. (e.g a billing date 1st Feb with cancellation notice on 15th Jan will result in 14 days pro-rated charge). Pro-rated charges are calculated as follows - (12 x monthly membership rate / 365) x days remaining exceeding next billing date.


Pre-Activity Readiness Questionnaire

Thank you for choosing to participate in our activities! To ensure your safety and well-being, we kindly request you to complete this Pre-Activity Readiness Questionnaire before engaging in any activities organised by CHALK W1. By completing a booking at CHALK, via, you are confirming the following statements: 

  1. I do not have or ever experienced any medical conditions or injuries that could affect your ability to participate in physical activities.

  2. I am not currently taking any medications that may impact your ability to engage in physical activities. 

  3. I do not have any allergies, especially those related to environmental factors or materials used during our activities.

  4. I have not had any recent surgeries or medical procedures that may affect your participation in physical activities.

  5. I have never been diagnosed with any heart conditions, high blood pressure, or other cardiovascular issues.

  6. I am not pregnant or planning to become pregnant in the near future. 

  7. I have not been advised by a healthcare professional to avoid any physical activities or have been given any specific restrictions.

  8. I have no known medical conditions or family history of medical conditions that we should be aware of for emergency purposes.

  9. I do not have any existing physical injuries, joint issues, or musculoskeletal concerns that might impact your participation in our activities. 

  10. I have not undergone any physical therapy or rehabilitation recently. 

Please note that your participation in our activities is voluntary, and by submitting this questionnaire, you acknowledge that you have provided accurate information to the best of your knowledge. If you experience any changes in your health status before the activity date, kindly inform us immediately.

By participating in our activities, you agree that CHALK W1, the Zedwell Hotel and their representatives are not liable for any injury, harm, or medical condition that may arise as a result of inaccurate information provided in this questionnaire or your participation in the activity.
